12 Months:     Polio, Prevnar, ProQuad (MMR & Varicella), Hepatitis A

15 Months:     HIB

18 Months:     DTAP, Hepatitis A

5 Years:        DTAP, IPV, ProQuad (MMR & Varicella)

12 Years:    Tdap, Menactra, Gardasil (girls only)

Yearly:    Influenza - 6 months to 5 years and selected patients of all ages
